Wellness Nurse-LPN
About The Position
Physicians’ Primary Care of Southwest Florida is a premier multi-specialty practice with locations in Cape Coral, Estero, Fort Myers, and Lehigh Acres. We are currently seeking a Wellness Nurse-LPN for our Adult Medicine Division. Our Wellness Nurses: Assist and support the physicians in providing annual wellness visits to our patients. Create and/or update personalized prevention plans to meet our patient's health goals. Educate, counsel, and track the progress of patients in a wellness therapy program. Maintain the strictest confidentiality and adheres to all HIPAA guidelines and regulations.
Requirements
- Minimum of three (3) years of clinical experience.
- Ability to self-direct and work independently and function well as a part of a team.
- Ability to handle all interactions in a professional and friendly manner.
- Excellent customer service skills.
- Prior experience in the creation and implementation of patient care programs.
- Graduate of an accredited nursing program and holds a valid and current Florida Licensed Practical Nurse (LPN) License.
