About The Position

The Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Wellness Navigator is responsible for working closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy, and support primarily in home and/or in the community. The Wellness Navigator will engage, educate, and offer support to individuals, their family patients, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.

Requirements

Highschool diploma or GED required.
Current AZ Driver’s License (vali and in good standing)
Clear 39-month Motor Vehicle Record (MVR).
Arizona Fingerprint Clearance card required

Nice To Haves

Associates degree (or higher) in a field related to behavioral health preferred.
Education and/or experience in the field of health and nutrition preferred.

Responsibilities
