The Wellness Nurse is responsible for providing nursing triage, health education, and risk reduction strategies to maximize the resident’s health and quality of life as defined by the 8 Dimensions of Wellness. This role involves assisting residents in achieving their health and wellness goals, providing support strategies and referrals for services to improve resident outcomes for chronic disease management and acute illness or injury, and acting as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle. The Wellness Nurse will develop a referral network of service providers, participate in collaborative care meetings, and communicate with the multi-disciplinary team to strategize for optimal resident outcomes. Responsibilities also include overseeing post-acute discharge planning, coordinating with health center and hospital staff, attending to medical emergency calls, completing incident reports, providing annual updates for medical demographic information and advance directives, organizing vaccination clinics, encouraging utilization of on-site healthcare practitioners, and partnering to plan and evaluate Wellness Programming. The role requires building relationships with residents, communicating effectively with families and staff, fostering measures to improve community culture, maintaining confidential records, attending training, and striving for high resident satisfaction. Sound judgment and strong organizational skills are essential. Other duties may be assigned by the Resident Services Director.
