About The Position

The Wellness Liaison is responsible for creating an excellent customer experience and will extend the activities of a Manager of Community Partnerships. Some of the key responsibilities will be outreach and engaging prospective participants, conducting comprehensive and accurate intakes onsite at key accounts and providing educational workshops. Position will be responsible for working to identify and assist prospective participants with procuring documentation needed, follow up with intake appointment and follow through at various stages of intake. The Wellness Liaison will continue to maintain contact with the clients until he/she is transitioned to Outreach program/service. The Wellness Liaison is an Outreach Ambassador and will excel in exemplifying the core values.

Requirements

  • High School Diploma/GED Equivalent required
  • 1 -2 years of work experience
  • Demonstrated ability to work as a mutually supportive team member as well as take independent initiative.
  • Evening and weekend work may be required.

Nice To Haves

  • Bachelor’s Degree a plus
  • Knowledge and experience working with at risk populations a plus
  • Bilingual English/Spanish preferred
  • Driver’s license preferred.

Responsibilities

  • Embody Outreach core values and be an Ambassador to carry out agency mission.
  • Extend activities of Manager of Community Partnerships in an account by providing intakes and preparing and providing educational workshops.
  • Maintain professional presence inside the assigned referral sources/accounts and provide excellent customer service.
  • Provide onsite screenings/assessments/intake at hospitals, courts, housing sites and other locations as needed.
  • Work with Manager of Community Partnerships to identify potential clients for Outreach programs and services.
  • Conduct comprehensive intake and complete required documentation.
  • Achieve monthly/ quarterly targets for Wellness Liaison.
  • Follow up with Outreach assigned programs to ensure all required information for intake is complete.
  • Daily record all intakes into Salesforce.
  • Maintain contact with the referred participant until the admission is complete and there is a warm transition to Outreach program.
  • Act as a liaison between external referral sources and internal Outreach programs to ensure internal customer understands account needs. Work with internal programs to ensure referrals/admissions are expedited; assists in getting any necessary documentation or information needed to process referrals.
  • Organize and deliver quality presentations in front of small and large groups.
  • Assist with health fairs and other outreach activities as needed.
  • May act as a direct point of contact with Outreach Departments on behalf of Business Development.
  • Active member of BD team, respectful of other team members and participates in team’s growth and development including onboarding new BD Team members.
  • Participate in Business Development team meetings, huddles calls, training and master basics on building long term relationships.
  • Complete required documentation to include Weekly Activity Report.
  • Work as part of Outreach team to support organizational growth and development.
  • Participate in implementation of client engagement/ retention programs.
  • Respond to inquiries from referrers, clients and others to assist with complaint resolution, explanation of Programs, and community relations.
  • Other special projects and duties to be assigned as necessary.
