About The Position

The primary purpose of the Wellness Director is to oversee the day-to-day wellness and resident care operations of Meadowbrook at Black River Falls' 24/7 Residential Care Apartment Complex (RCAC), serving up to 30 residents. The Wellness Director provides leadership and support to the Resident Assistant team, promotes high-quality resident care, ensures compliance with applicable regulations and Meadowbrook policies, and works closely with residents, families, providers, and the leadership team to meet the changing needs of our residents. This is a hands-on leadership position that requires strong communication, organization, clinical judgment, and the ability to build positive relationships with both residents and team members.

Requirements

  • Current, unencumbered Wisconsin RN or LPN license in good standing.
  • Must be able to read, write, speak, and understand English.
  • Must demonstrate professionalism, compassion, patience, and respect when working with residents, families, and team members.

Nice To Haves

  • Previous leadership, supervisory, or healthcare management experience preferred.
  • Experience in assisted living, RCAC, long-term care, senior living, or a similar healthcare setting preferred.
  • Strong knowledge of resident care practices, medication management, and applicable regulations.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time-management skills.
  • Ability to make sound decisions and respond appropriately to changing resident needs.

Responsibilities

  • Oversee the day-to-day wellness operations of the RCAC and ensure residents receive safe, compassionate, and consistent care.
  • Monitor residents' physical, emotional, and overall well-being and respond appropriately to changes in condition.
  • Participate in resident assessments and the development, implementation, and ongoing review of individualized service and care plans.
  • Ensure Resident Assistants understand and follow each resident's care plan and individual needs.
  • Communicate with residents, families, physicians, pharmacies, and other healthcare providers as needed.
  • Coordinate follow-up when there are changes in a resident's condition, medical appointments, hospitalizations, or other healthcare needs.
  • Make regular rounds throughout the community and remain visible and accessible to residents and staff.
  • Address resident and family concerns promptly, professionally, and compassionately.
  • Promote resident dignity, privacy, independence, choice, and rights in all aspects of care.
  • Provide hands-on resident care when needed to support the team and meet resident needs.
  • Lead, support, coach, and develop the Resident Assistant team.
  • Ensure appropriate staffing and coverage across all shifts in a 24/7 environment.
  • Assist with scheduling and staffing adjustments based on resident needs and department needs.
  • Monitor attendance, performance, and adherence to Meadowbrook policies and expectations.
  • Participate in hiring, interviewing, onboarding, orientation, and training of Resident Assistants.
  • Provide ongoing coaching, feedback, and support to team members.
  • Address performance concerns appropriately and partner with leadership on disciplinary matters when needed.
  • Promote teamwork, accountability, communication, and a positive work environment.
  • Maintain a calm, respectful, and welcoming environment for residents, families, and staff.
  • Oversee medication-related processes within the RCAC in accordance with applicable regulations, resident service plans, and Meadowbrook policies.
  • Ensure medication systems and documentation are completed accurately and consistently.
  • Monitor medication administration and delegation practices as applicable.
  • Complete or coordinate medication audits and competency checks as required.
  • Ensure Resident Assistants complete required training and competencies before performing assigned responsibilities.
  • Maintain accurate and timely resident documentation.
  • Ensure the community remains prepared for regulatory surveys and inspections.
  • Participate in quality assurance activities and assist with identifying and correcting areas of concern.
  • Maintain confidentiality of resident information and protect all resident health information.
  • Develop and maintain an effective orientation process for new Resident Assistants.
  • Provide ongoing education, coaching, and in-service training related to resident care, safety, infection prevention, emergency procedures, and Meadowbrook policies.
  • Identify training needs and provide additional education or coaching when concerns are identified.
  • Encourage team members to continue developing their skills and knowledge.
  • Ensure staff understand their responsibilities and are equipped to provide safe and appropriate resident care.
  • Maintain open communication with the Administrator and other members of the leadership team.
  • Work collaboratively with families, residents, healthcare providers, pharmacies, and community partners.
  • Participate in care conferences, quality assurance meetings, and other meetings as needed.
  • Communicate important resident and staffing information to the appropriate leadership team members.
  • Respond appropriately to resident complaints, grievances, incidents, and concerns.
  • Assist with investigations and follow-up related to resident care, safety, abuse, neglect, or other concerns.
  • Promote a safe environment for residents, visitors, and team members.
  • Ensure staff follow established infection prevention and control practices.
  • Ensure appropriate use of PPE and other safety equipment.
  • Monitor the community for potential safety concerns and report or address issues appropriately.
  • Participate in emergency preparedness activities and drills.
  • Assist with emergency situations and resident evacuation when necessary.
  • Ensure resident care areas and wellness supplies are maintained in a safe and sanitary condition.
  • Ensure residents are treated with dignity, kindness, respect, and compassion.
  • Protect resident privacy and confidentiality.
  • Support residents' rights to make choices regarding their care and daily lives.
  • Ensure residents are informed of their rights and that their preferences are respected.
  • Respond to resident and family concerns in a timely and professional manner.
  • Report and appropriately respond to suspected abuse, neglect, exploitation, or misappropriation of resident property.
  • Maintain accurate records, reports, and documentation related to the wellness department.
  • Monitor wellness supplies and communicate department needs to the Administrator.
  • Assist with managing department resources and expenses.
  • Review wellness policies and procedures and recommend updates as needed.
  • Participate in facility surveys and inspections.
  • Assist with quality improvement initiatives and follow through on corrective actions.
  • Participate in the Manager on Duty/on-call rotation as assigned.
  • Perform other duties as assigned by the Administrator.
