About The Position

Fox Ridge NLR, now part of the AgeWell Solvere Living family, is seeking a Wellness Director. This role reports to the Executive Director and is responsible for the administration of nursing services. The Wellness Director will direct, plan, and coordinate the services and activities of professional nursing and auxiliary nursing personnel in rendering resident care. This includes interpreting policy and regulations to nursing personnel and ensuring compliance. Responsibilities involve analyzing and evaluating nursing services, overseeing resident healthcare needs, managing the medication delivery system, and coordinating in-service training for the Wellness department. The role contributes to a team approach to resident care by addressing physical, emotional, and social needs to promote resident independence. Ensuring clinical services comply with state regulations, accepted clinical standards, and Community policies is crucial. The position also involves coordinating and managing nursing and personal care associate services, providing direct nursing services, and ensuring regulatory compliance with assisted living/memory care laws. Key duties include managing the move-in process, conducting wellness reviews and plan development, coordinating resident healthcare with external agencies, conducting wellness plan reviews with residents and families, and supervising staff training and education. The Wellness Director is also responsible for infection control, managing move-outs, quality assurance through auditing, preparing staffing schedules and reports, and following emergency and safety procedures.

Requirements

  • Must be a Registered Nurse or a Licensed Practical/Vocational Nurse licensed to practice in the State
  • A baccalaureate degree in nursing and a minimum of two (2) years of full time, or the equivalent of full-time, clinical experience in nursing, at least one year of which shall be in a home healthcare agency or Community health program that included care of the sick at home; OR A diploma or associates degree in nursing and at least four (4) years of full-time, or the equivalent of full-time, clinical experience in nursing within the past ten (10) years, at least (1) year of which shall be in a home healthcare agency or Community health program that included care of the sick at home.
  • Experience and passion in working with seniors
  • Able to perform tasks which may be physically demanding such as pushing, bending, and lifting up to 60 pounds
  • Able to maintain positive working relationships with Residents, their families, peers, and other staff members
  • In good physical and emotional health and free of communicable diseases
  • Able to perform frequent lifting, pushing, pulling, moving of equipment, bending, and stooping
  • Able to spend long periods of time on foot
  • Able to react quickly in any situation

Responsibilities

  • Administration of nursing services
  • Directing, planning, and coordinating services and activities of professional nursing and auxiliary nursing personnel
  • Interpreting policy and regulations to nursing personnel and ensuring compliance
  • Analyzing and evaluating nursing and related services rendered to ensure quality of resident care
  • Oversight of resident healthcare service’s needs, including the overall medication delivery system
  • Coordination of in-service training for the Wellness department
  • Contributing directly and positively to the team approach to quality resident care by attending to and assisting with a variety of physical, emotional, and social needs to help residents maintain the highest level of independence possible
  • Ensuring clinical services are provided in accordance with state regulations, accepted clinical standards, and Community policies & procedures
  • Assisting in the coordination and management of nursing and personal care associate services provided to residents
  • Providing nursing services to residents: Resident Instruction, Wellness Counseling, Health Promotion, Disease Prevention
  • Providing care and services to residents whose conditions are chronic and stable
  • Verifying resident’s medication regimen with physicians or healthcare practitioners for Residents who require supervision
  • Providing assistance to Residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility, if needed
  • Ensuring and maintaining regulatory compliance with all federal and state laws and regulations regarding assisted living/memory care and the regulations of the state’s nursing commission
  • Assisting in the coordination and management of care and services for residents through the Move-In Process (Pre move-in wellness review, review of paperwork, and initial Resident Service Plan Development)
  • Identifying need for additional assessment, health monitoring, or other coordination of care needs
  • Initiating Resident Wellness Plan review process
  • Coordinating Resident healthcare in collaboration with residents, their families, and outside healthcare agencies, including referral when indicated
  • Conducting regular Wellness Plan Reviews with Resident families, including meetings with responsible parties and including appropriate staff whenever changes in care levels occur
  • Coordinating and participating in initial job-specific orientation and training through routine review of training documents, in accordance with community policies & procedures
  • Coordinating and conducting continuing training and education on a regular basis, as identified through routine auditing
  • Reviewing documentation and communication of Wellness Associates
  • Supervising assigned personnel in the delivery of nursing services and assistance with activities
  • Ensuring proper and accurate documentation
  • Providing RN delegation, if an RN, in accordance with state regulations and Community policy
  • Being available after-hours for issues related to current resident needs; may be required to return to the Community or to conduct offsite assessment at certain times
  • Maintaining compliance with Community policies on Immunizations, Infection Control, and Infectious Disease Management
  • Assisting Residents who no longer require services
  • Completing routine auditing of all resident care systems, in accordance with all QA tools
  • Preparing monthly Wellness staffing schedule
  • Providing weekly reports to General Manager regarding any issues associated with the provision of core services for the AgeWell Solvere Living Managed Community and preparing summaries to be provided to the governing authority, in accordance with the schedule established by the governing authority
  • Participating in other resident care services and community-related duties, as assigned by the Executive Director
  • Following all emergency procedures
  • Understanding all safety policies and procedures
  • Communicating effectively with Executive Director and other community team members
