About The Position

Heritage Senior Living is seeking an experienced and compassionate Wellness Director to lead the clinical and resident wellness operations of our community. Reporting to the Executive Director, the Wellness Director is responsible for resident care and wellness, regulatory compliance, clinical documentation, medication management, infection prevention, and the leadership and development of our nursing and caregiver teams. This is an opportunity for a nurse who wants to combine clinical expertise, leadership, and meaningful relationships with residents and families to help create an exceptional senior living experience.

Requirements

  • Degree from an accredited school of nursing required.
  • Current and valid Wisconsin nursing license in good standing required.
  • At least one year of nursing experience working with older adults required.
  • Strong knowledge of resident assessment, care planning, medication management, documentation, and clinical practices.
  • Strong clinical judgment, problem-solving, and decision-making skills.
  • Excellent leadership, interpersonal, organizational, and customer-service skills.
  • Ability to communicate complex clinical concerns clearly and professionally.
  • Ability to work independently while collaborating effectively with the Executive Director and interdisciplinary team.
  • Excellent written and verbal communication skills.
  • Caring and compassionate approach with genuine concern for older adults.
  • Ability to meet applicable pre-employment, training, licensing, and regulatory requirements.

Nice To Haves

  • Assisted living, memory care, long-term care, or similar senior-care experience preferred.
  • Skilled nursing facility (SNF) experience preferred.
  • Previous nursing leadership, supervisory, or management experience preferred.
  • Experience supervising and developing nurses, caregivers, CNAs, or other direct-care employees preferred.
  • Proficiency with Microsoft applications; PointClickCare or similar electronic health record experience preferred.

Responsibilities

  • Oversee the delivery and coordination of resident care and wellness services throughout the community.
  • Conduct new-resident interviews and assist with clinical data collection and required admission documentation.
  • Review medical provider information and resident needs prior to move-in.
  • Ensure the care team understands new residents' care needs, preferences, and special dietary requirements.
  • Conduct regular resident check-ins and identify ongoing needs that support health, safety, and wellness.
  • Meet with residents and families to discuss wellness needs, concerns, and changes.
  • Coordinate with physicians and other healthcare providers regarding resident health needs.
  • Track physician appointments and follow up on recommendations and orders.
  • Ensure changes in resident condition are appropriately evaluated, communicated, documented, and addressed.
  • Develop and maintain Individualized Service Plans (ISPs) according to applicable requirements and community procedures.
  • Complete ISPs within required timeframes following admission.
  • Review and update ISPs at least every six months and following significant changes in resident condition as required.
  • Conduct ISP meetings with residents, families, caregivers, nursing staff, and other appropriate members of the care team.
  • Ensure resident services accurately reflect identified needs, preferences, physician orders, and changes in condition.
  • Communicate ISP updates to employees responsible for resident care.
  • Monitor care delivery to help ensure services are consistent with each resident's plan.
  • Provide oversight of the community medication-management program.
  • Monitor medication administration practices, documentation, storage, and related procedures.
  • Conduct routine medication and MAR audits and ensure identified concerns are appropriately addressed.
  • Monitor resident medication schedules and help ensure adequate medication supplies are maintained.
  • Review and process incoming medication orders and changes.
  • Communicate medication and treatment changes to appropriate caregivers, CNAs, LPNs, and other clinical team members.
  • Provide or coordinate employee education related to medication administration.
  • Process MAR documentation and required resident summary notes.
  • Coordinate periodic review of psychotropic medications with appropriate healthcare providers and the resident's care team.
  • Promote safe medication practices consistent with applicable requirements and company procedures.
  • Provide clinical leadership, direction, and support to LPNs, caregivers, CNAs, and other assigned care team members.
  • Provide supervisory oversight to the LPN serving in a management role and support their continued leadership development.
  • Partner with the Executive Director to maintain appropriate care staffing and schedules based on resident and community needs.
  • Participate in recruiting, interviewing, onboarding, training, coaching, and development of care team employees.
  • Regularly assess caregiver/CNA skills and identify opportunities for additional training.
  • Establish clear expectations for resident care, documentation, medication practices, customer service, and professionalism.
  • Coach employees and address performance concerns in partnership with community leadership.
  • Promote accountability, teamwork, communication, and resident-centered care.
  • Serve as a clinical resource to employees throughout the community.
  • Develop and maintain working knowledge of applicable federal, Wisconsin, and company requirements, policies, and procedures.
  • Help ensure community clinical operations remain in compliance with applicable requirements.
  • Maintain accurate, complete, and timely resident clinical documentation.
  • Review daily clinical logs and promptly follow up on incidents, concerns, or resident changes.
  • Monitor accidents and incidents and ensure appropriate documentation and follow-up.
  • Assist with OSHA and workplace-safety compliance responsibilities.
  • Prepare for and participate in regulatory surveys, inspections, audits, and other compliance activities.
  • Identify compliance concerns and implement appropriate corrective action in partnership with community and corporate leadership.
  • Monitor infections and infection trends throughout the community.
  • Ensure employees follow established infection-prevention and infection-control procedures.
  • Provide education and follow-up when opportunities for improvement are identified.
  • Review resident incidents and changes for potential clinical or safety concerns.
  • Promote practices that protect resident health, safety, dignity, independence, and quality of life.
  • Collaborate with community leadership to identify and address resident-care risks.
  • Develop strong relationships with residents and their families.
  • Serve as a clinical resource for resident and family questions and concerns.
  • Communicate professionally with physicians, pharmacies, hospitals, hospice providers, home health providers, and other healthcare partners.
  • Act as a liaison between residents, families, healthcare providers, and the community care team.
  • Help families understand changes in resident needs and services.
  • Promote clear, timely, and compassionate communication.
  • Partner closely with the Executive Director on clinical operations, staffing, resident needs, and community priorities.
  • Participate in leadership and care coordination meetings.
  • Assist with prospective resident inquiries and community tours as needed.
  • Provide clinical input during the prospective-resident and move-in process.
  • Collaborate with culinary, life enrichment, housekeeping, maintenance, and other departments to support resident wellness.
  • Participate in on-call leadership responsibilities and provide clinical guidance when needed.
  • Perform other clinical and leadership responsibilities appropriate to the role.
