Requirements

  • Must be a Licensed Practical/Vocational Nurse licensed to practice in the State.
  • Either: A baccalaureate degree in nursing and a minimum of two (2) years of full time, or the equivalent of full-time, clinical experience in nursing, at least one year of which shall be in a home healthcare agency or Community health program that included care of the sick at home; OR A diploma or associates degree in nursing and at least four (4) years of full-time, or the equivalent of full-time, clinical experience in nursing within the past ten (10) years, at least (1) year of which shall be in a home healthcare agency or Community health program that included care of the sick at home.
  • Experience and passion in working with seniors.
  • Able to perform tasks which may be physically demanding such as pushing, bending, and lifting up to 60 pounds.
  • Able to maintain positive working relationships with Residents, their families, peers, and other staff members.
  • In good physical and emotional health and free of communicable diseases.
  • Able to perform frequent lifting, pushing, pulling, moving of equipment, bending, and stooping.
  • Able to spend long periods of time on foot.
  • Able to react quickly in any situation.

Responsibilities

  • Administer nursing services.
  • Direct, plan, and coordinate the services and activities of professional nursing and auxiliary nursing personnel in rendering resident care.
  • Interpret policy and regulations to all nursing personnel and ensure compliance.
  • Analyze and evaluate nursing and related services rendered to ensure quality of resident care.
  • Oversee resident healthcare service’s needs, including the overall medication delivery system and the coordination of in-service training for the Wellness department.
  • Contribute directly and positively to the team approach to quality resident care by attending to and assisting with a variety of physical, emotional, and social needs to help residents maintain the highest level of independence possible.
  • Ensure clinical services are provided in accordance with state regulations, accepted clinical standards, and Community policies & procedures.
  • Assist in the coordination and management of nursing and personal care associate services provided to residents.
  • Provide nursing services to residents: Resident Instruction, Wellness Counseling, Health Promotion, Disease Prevention.
  • Provide care and services to residents whose conditions are chronic and stable.
  • Verify resident’s medication regimen with physicians or healthcare practitioners for Residents who require supervision.
  • Provide assistance to Residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility, if needed.
  • Be aware and knowledgeable of the federal and state laws and regulations regarding assisted living/memory care and the regulations of the state’s nursing commission; ensure and maintain regulatory compliance with all such regulations.
  • Assist in the coordination and management of care and services for residents through the following: Move-In Process (Pre move-in wellness review, review of paperwork, and initial Resident Service Plan Development), Wellness Review and Plan (Identifies need for additional assessment, health monitoring, or other coordination of care needs. Initiates Resident Wellness Plan review process).
  • Coordinate Resident healthcare in collaboration with residents, their families, and outside healthcare agencies, including referral when indicated.
  • Conduct regular Wellness Plan Reviews with Resident families, including meetings with responsible parties and including appropriate staff whenever changes in care levels occur.
  • Coordinate and participate in initial job-specific orientation and training through routine review of training documents, in accordance with community policies & procedures.
  • Coordinate and conduct continuing training and education on a regular basis, as identified through routine auditing.
  • Review documentation and communication of Wellness Associates.
  • Supervise assigned personnel in the delivery of nursing services and assistance with activities.
  • Ensure proper and accurate documentation.
  • Be available after-hours for issues related to current resident needs; may be required to return to the Community or to conduct offsite assessment at certain times.
  • Maintain compliance with Community policies on Immunizations, Infection Control, and Infectious Disease Management.
  • Assist Residents who no longer require services.
  • Complete routine auditing of all resident care systems, in accordance with all QA tools.
  • Prepare monthly Wellness staffing schedule.
  • Provide weekly reports to General Manager regarding any issues associated with the provision of core services for the AgeWell Solvere Living Managed Community and prepares summaries to be provided to the governing authority, in accordance with the schedule established by the governing authority.
  • Participate in other resident care services and community-related duties, as assigned by the Executive Director.
  • Follow all emergency procedures.
  • Understand all safety policies and procedures.
  • Communicate effectively with Executive Director and other community team members.
