About The Position

Spring Oak Senior Living at Warrenton is seeking an experienced Licensed Practical Nurse (LPN) to serve as the Wellness Director for our assisted living community. Spring Oak Senior Living is a family-owned and operated senior living company. Requirements

  • Current Licensed Practical Nurse (LPN) license in Virginia required.
  • At least 2 years of nursing leadership, supervisory, or management experience required.
  • Strong knowledge of resident assessments, care planning, medication management, clinical documentation, and regulatory compliance.
  • Strong leadership, communication, organization, and follow-through skills.
  • Ability to build positive relationships with residents, families, staff, physicians, and outside providers.

Nice To Haves

  • Experience in assisted living, senior living, memory care, long-term care, skilled nursing, or residential care strongly preferred.
  • Experience with eMAR or EHR systems preferred

Responsibilities

  • Lead, supervise, coach, and support CNAs, CMAs, medication aides, caregivers, and personal care staff.
  • Oversee resident assessments, service plans, care plans, and wellness services.
  • Conduct and oversee resident assessments, including assessments for prospective and new residents, and help determine appropriate care needs, service plans, and readiness for move-in.
  • Manage eMAR documentation, medication administration oversight, pharmacy coordination, and medication-related compliance.
  • Monitor resident changes in condition and ensure proper follow-up, documentation, and communication.
  • Ensure compliance with Virginia assisted living regulations, HIPAA, infection control standards, and company policies.
  • Coordinate care with physicians, hospitals, hospice, home health, rehabilitation providers, pharmacies, and families.
  • Assist with hiring, training, scheduling, coaching, and performance management for the care team.
  • Maintain accurate clinical documentation and prepare for surveys, audits, and state inspections.
  • Monitor resident safety, incident reports, fall management, infection control, and quality assurance.
  • Participate in the required on-call rotation and respond to clinical concerns or emergencies as needed.
