Wellness Coordinator (LPN or RN) (part-time)
About The Position
Community Living New Horizons is looking for a part-time Wellness Coordinator (Nurse) to assist adults with intellectual and developmental disabilities. The role involves helping individuals reach their goals, grow, develop skills, and live as independently as possible in the community. The Wellness Coordinator is responsible for developing, maintaining, and monitoring individual wellness plans and ensuring the medical services needed to manage healthcare needs are in place. This role requires excellent communication and interpersonal skills to build positive relationships with the individuals supported, as well as critical thinking, time management, and organizational skills. The Wellness Coordinator makes a positive difference in the lives of those they support.
Requirements
- Nursing degree from an accredited school
- Valid nursing license (RN or LPN)
- Valid Driver’s License
- Completion of required training including First Aid/CPR certification
Responsibilities
- Development, maintenance, and routine monitoring of the individual waiver participant’s wellness plan.
- Ensuring the medical services required to manage the health care needs of the individual are in place.
