About The Position

This position is for a Wellness Coach (Case Manager) who has a passion to help others and transform their lives. The role involves assisting consumers with accessing needed community services such as medical, psychological, substance abuse, vocational, food, and clothing through the development of a referral network. Requirements

Bachelor’s Degree in Human Services field required.
Two years experience with individuals with mental illness, substance abuse issues and/or a history of homelessness required.
Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System.

Nice To Haves

Experience in scattered site setting strongly preferred.
Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred.
Experience or training in peer counseling or advocacy preferred.
Bilingual English/Spanish preferred.
Valid NYS driver’s license preferred.

Responsibilities

Engage people with mental illness and assist them in meeting the demands of community living and maintaining their housing by teaching them the necessary skills, and linking them to services that will support their recovery, independence and self-determination.
Conduct Group Meetings on Wellness Management.
Complete service plan reviews for all residents.
Regularly assess, correct and report apartment and personal living space inspections conditions as assigned.
Provide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated.
