About The Position

Under the supervision of the Executive Director, the Wellness Coach will work closely with Regional Health Education as an extension of the care teams supporting patient's goals. This position works independently providing coaching, educational support, resource navigation and troubleshooting by phone, in person, group and individual settings. In addition, a regional resource for both patients and employees navigating all wellness/disease management programs within or outside St. Joseph Hoag Health. Partner with SJHH Employee Wellness program to extend the education/disease management services at the employee worksite. Providence caregivers are not simply valued – they’re invaluable. Requirements

  • Bachelor's Degree Health education, health promotion, nursing, athletic training, nutrition, or other related field.
  • 2 years of Health and wellness field.
  • Teaching and modeling self-management skills, concepts, resources and tools related to health, lifestyle change and behavior modification.
  • Direct member communication.
  • Working with patients with chronic disease.
  • Background in specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, stress reduction and chronic conditions.
  • Program planning, implementation, evaluation, and education training.

Nice To Haves

  • Master's Degree Health education, health promotion, nursing, athletic training, nutrition, or other related field.
  • Certification from an accredited professional Health Coach Training Program upon hire
  • CHES (Certified Health Education Specialist) upon hire
  • 3 years of Health coaching experience.
  • Excel, PowerPoint, Access, and Outlook experience.
