Requirements

  • Master’s degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program
  • LSW or LAC required
  • Valid driver’s license required
  • No more than one moving violation in the past twelve months
  • Vehicle required

Nice To Haves

  • CPRP preferred
  • Minimum of one-year experience working with persons with mental health disabilities preferred

Responsibilities

  • Creates and Maintains a wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning. All services provided will promote wellness and recovery outcomes.
  • Creates and Maintains a culture of compliance with internal and external policies, regulations, laws and high ethical standards.
  • Under the clinical supervision of an LCSW or LPC is able to complete the Comprehensive Rehabilitation Needs Assessment on an ongoing basis and also contribute to its development.
  • Under the clinical supervision of an LCSW or LPC is able to develop, implement, monitor and update the Individualized Recovery Plan/Individual Recovery Plan (IRP).
  • Offer services appropriate to meet person’s served needs as identified in the Comprehensive Rehabilitation Needs Assessment (CRNA) and included in each individuals IRP.
  • Perform therapeutic rehabilitative skill and resource development as outlined in the CSS Regulations.
  • Perform crisis intervention as indicated.
  • Coordinate and manage services in conjunction with the team.
  • Utilizes WRAP and other relevant health and wellness curriculum to ensure overall wellness is a key focus in the individual’s recovery plan.
  • Perform evidence-based practices that are appropriate for implementing community support services. This may include Motivational Interviewing, Cognitive Behavioral and Educational Strategies, Trauma Informed Care, Permanent Supportive Housing, IMR, and Career Services.
  • Mental health education, skill training and supportive counseling in the home and other natural environments to persons served and significant others.
  • Utilizes a clinical skill-set by forging therapeutic relationships with diverse individuals through a trauma-informed and recovery-based lens.
  • In conjunction with the team leader and Wellness Clinician LCSW or LPC, the LSW or LAC is able to provide administrative supervision of the daily tasks.
  • Weekly individual clinical supervision is required in addition to administrative supervision at least monthly.
  • Coordinates referrals and linkages to housing, mental health, substance abuse; medical and various social service providers as needed for the individual.
  • Crisis intervention services for persons served with acute psychiatric needs whose circumstances and conditions require rapid intervention.
  • Provides stages wise treatment utilizing motivational interviewing and cognitive behavioral techniques.
  • Cognitive behavioral techniques include: modeling, role playing, relapse prevention, relaxation, encouragement and reinforcement will be used to teach and normalize community interaction.
  • 24 hour on-call coverage on a rotating schedule.
  • Outreach to newly enrolled individuals and encourages those reluctant to accept mental health services through interventions with them and/or family members and friends concerned with their welfare.
  • Facilitates weekly team meetings for scheduling in the absence of the team leader ensuring completion of daily contact log.
