The Well-being Coach utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage and promote members in decisions related to achieving and maintaining their optimal well-being. The goal is to support members in identifying and achieving their own personal goals across any of the six dimensions of wellness including physical, emotional, social, financial, purpose, and character strengths. In utilizing motivational interviewing and active listening skills, this person provides coaching and support to empower the member to meet their personal wellness goals and sustain long-term positive behavioral change. The role also includes assessing a member’s appropriateness for coaching and coordination into other levels of care and/or resources as needed. Additionally, the Well-being Coach develops and monitors established plans of care, in collaboration with the member, in order to meet the member’s goals.
