About The Position

Welcome Home RN - Start of Care & Admissions for Home Health. At Iowa Home Care, we don't call it our Intake Department. We call it our Welcome Home Department - because we believe the beginning of home health care should feel exactly that way. We're looking for an experienced, compassionate Registered Nurse to join our Welcome Home team and specialize in Start of Care admissions for clients throughout the Des Moines area. This is a great opportunity for an RN who loves assessment, OASIS, care planning, patient education, and putting all the pieces together - but may be ready for something different from a traditional ongoing case-management role. Your specialty will be getting care started right. This isn't traditional bedside nursing, and it isn't traditional home health case management. As a Welcome Home RN, you'll specialize in one of the most important parts of the home health journey: getting care started correctly. You'll meet new clients and families, assess the whole clinical picture, build the roadmap for their care, and then partner with the appropriate clinical team members to carry that plan forward. You'll have independence in the field while remaining closely connected to an experienced clinical team. Requirements

Current Iowa Registered Nurse (RN) license
Strong clinical assessment and critical-thinking skills
Excellent communication and organizational skills
Ability to work independently and manage your day effectively
Commitment to timely, accurate, high-quality clinical documentation
A genuine desire to make clients and families feel comfortable and confident receiving care at home

Nice To Haves

Previous home health experience
Previous OASIS experience
Experience completing Start of Care assessments
Knowledge of Medicare home health requirements
Admissions, care-transition, discharge-planning, or case-management experience

Responsibilities

Complete comprehensive Start of Care (SOC) admissions for new Iowa Home Care clients.
Complete OASIS assessments and required admission documentation.
Perform comprehensive nursing assessments and medication reconciliation.
Identify clinical, functional, safety, social, and support needs.
Get to know each client and family, including their goals, priorities, and what matters most to them.
Develop individualized plans of care designed to help clients safely and successfully remain at home.
Coordinate with physicians and other providers to obtain necessary orders.
Identify appropriate disciplines, services, visit frequencies, and clinical priorities.
Educate clients and families about their care and what they can expect from Iowa Home Care.
Collaborate with nursing, therapy, social work, home health aides, scheduling, and clinical leadership.
Provide a thorough clinical handoff to the RN Case Manager and other clinicians who will continue the client's care.
Help ensure every new client has a smooth transition into Iowa Home Care.
