About The Position

The Weekend Visit Nurse provides hospice and palliative care to patients in their places of residence in Montgomery County. The nurse assesses patients’ needs, serves as a clinical resource for referral sources and patients and family members, and facilitates end-of-life conversations and decision-making with families or caregivers. The nurse also represents JSSA in communications with patients, families, and physicians. The schedule is every other weekend, Saturdays and Sundays, from 8:30 a.m. to 5:00 p.m.

Requirements

Active State of Maryland RN or LPN license.
At least 3 years of acute care and/or hospice experience.
Knowledge and skills necessary to provide meaningful care and communicate with terminally ill patients.
Excellent communication and customer service skills.
Attention to detail and strong organizational skills.
Proficient computer skills.
Valid driver’s license and reliable, independent transportation to provide care to hospice patients in Montgomery County.
Availability to work the assigned weekend shift.

Nice To Haves

CHPN or CHPLN certification preferred.
Prior hospice experience preferred.
Familiarity with Netsmart My-Unity electronic medical records is a plus.
