About The Position

Interim HealthCare of Oklahoma City is hiring a Weekend Triage Nurse to be the first call for our home health patients on Saturdays and Sundays. When a patient or family calls, you'll assess the situation by phone, give clear guidance, and dispatch a field nurse when a visit is needed. This role is phone-based from home, with on-call field nurses making any necessary visits. The agency is locally owned, Veteran- and nurse-owned, and offers a steady clinical team to support the nurse.

Requirements

• Active Oklahoma RN license
• Home health experience is required
• Previous triage or on-call experience in home health is required
• Sound clinical judgment and the confidence to make decisions over the phone
• Strong knowledge of home health patient populations, plans of care, and when to escalate
• Calm, clear communication with patients, families, nurses, and physicians
• Reliable phone and internet access while on shift

Responsibilities

• Take incoming weekend calls from home health patients, families, and caregivers
• Assess symptoms and concerns by phone and decide on the right next step
• Dispatch the on-call field nurse when a visit is needed and give them what they need to know before they go
• Contact physicians for orders or updates when a patient's condition changes
• Give patients and families clear instructions and reassurance when a visit isn't needed
• Document every call accurately and on time in our EMR
• Hand off weekend activity to the clinical team on Monday so nothing falls through the cracks
