About The Position

Hollenbeck Palms is seeking a compassionate, detail-oriented, and clinically skilled Licensed Vocational Nurse (LVN) to serve as a Weekend Treatment Nurse in its 59-bed Skilled Nursing Facility. The Weekend Treatment Nurse is responsible for providing and coordinating ordered treatments, wound care, skin assessments, and related clinical documentation while supporting resident comfort, safety, dignity, and quality of care. This position works closely with the Director of Nursing, charge nurses, interdisciplinary team members, attending physicians, and outside clinical providers to ensure timely follow-up of treatment orders and changes in resident condition.

Requirements

  • Current and active California Licensed Vocational Nurse license in good standing.
  • Current CPR/BLS certification.
  • Knowledge of skilled nursing regulations, infection prevention standards, resident rights, and documentation requirements.
  • Ability to assess resident needs, recognize changes in condition, exercise sound clinical judgment, and escalate concerns appropriately.
  • Strong organizational, time-management, communication, and interpersonal skills.
  • Ability to work independently during weekend operations while collaborating effectively with the nursing team and on-call leadership.
  • Ability to stand, walk, bend, reach, and perform resident-care activities throughout the shift.
  • Ability to safely assist with resident positioning, transfers, and mobility in accordance with resident needs and facility safe-lifting practices.
  • Ability to lift, carry, push, and pull supplies or equipment as needed, with or without reasonable accommodation.
  • Ability to work in an environment that may involve exposure to infectious diseases, bodily fluids, and other healthcare-related hazards, using appropriate protective equipment and safety procedures.

Nice To Haves

  • Minimum one year of nursing experience in a skilled nursing facility, long-term care, rehabilitation, wound care, or similar clinical setting preferred.
  • Experience with wound care, skin assessments, treatment documentation, and electronic health records strongly preferred.

Responsibilities

  • Perform and document ordered treatments in accordance with physician orders, care plans, facility policies, and applicable state and federal regulations.
  • Provide wound care and skin treatments, including dressing changes, pressure injury prevention interventions, monitoring of skin integrity, and related documentation.
  • Complete weekly skin assessments and focused skin assessments as assigned or when a change in condition is identified.
  • Accurately measure, photograph when appropriate and authorized, monitor, and document wounds, skin concerns, treatment progress, and resident response to treatment.
  • Promptly notify the Charge Nurse, RN Supervisor, Director of Nursing, physician, responsible party, and other appropriate team members of significant changes in a resident’s wound status, skin integrity, pain level, infection concerns, or overall condition.
  • Review treatment administration records, physician orders, wound care recommendations, and care plans to ensure treatments are completed accurately and timely.
  • Assist with implementation of treatment orders received from physicians, wound care specialists, therapy providers, and other authorized clinical professionals.
  • Monitor treatment supplies and communicate supply needs to the appropriate supervisor to support continuity of care.
  • Participate in infection prevention practices, including appropriate use of personal protective equipment, hand hygiene, isolation precautions, and reporting of infection-control concerns.
  • Collaborate with nursing staff to reinforce treatment schedules, skin integrity interventions, repositioning needs, and resident-specific care requirements.
  • Provide clinical guidance and education to Certified Nursing Assistants and other care staff within the LVN scope of practice regarding skin care, pressure injury prevention, and treatment-related resident needs.
  • Support admissions, readmissions, incident follow-up, and other nursing duties when assigned and within the scope of licensure.
  • Maintain complete, timely, accurate, and legible clinical documentation in the electronic health record.
  • Report suspected abuse, neglect, exploitation, injuries of unknown origin, or other reportable concerns immediately in accordance with facility policy and regulatory requirements.
  • Maintain resident confidentiality and comply with HIPAA, Hollenbeck Palms policies, and all applicable privacy requirements.
  • Attend required in-services, staff meetings, and training programs.
  • Perform other related duties as assigned by the Director of Nursing, Assistant Director of Nursing, or designee.
