Requirements

Current, active LPN license in the state of practice
At least 2 years of nursing experience in skilled nursing, long-term care, or a similar healthcare setting preferred
Strong knowledge of long-term care regulations and resident care planning
Excellent communication, leadership, and organizational skills
Ability to prioritize, problem solve, and work independently while collaborating with the interdisciplinary team

Responsibilities

Coordinate and oversee resident care plans to ensure each resident receives individualized, high-quality care
Monitor resident conditions, complete assessments, and communicate changes with physicians and the interdisciplinary team
Provide leadership, support, and guidance to nursing staff while promoting a positive team environment
Serve as a trusted resource for residents and their families by providing updates and addressing concerns
Ensure accurate documentation and compliance with state, federal, and facility regulations
Participate in quality assurance activities and help maintain excellence in resident care
