Requirements

Current RN licensure in the state of Kentucky and/or Indiana, or the ability to obtain required license/s upon offer of position.
Valid driver’s license.

Nice To Haves

BSN desired.
Certification in hospice/palliative care preferred.
One year of nursing experience preferred.
Knowledge of palliative care helpful.
Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

Assess and identify patient/family needs.
Plan, implement, evaluate and document individualized hospice nursing care.
Provide care in accordance with the interdisciplinary plan of care and organizational policies.
Coordinate care and collaborate among the Interdisciplinary Group and other medical providers.
Supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.
Make visits to patients in various settings including home, skilled nursing facility, assisted living facility, or acute care.
Conduct admission visits for medically eligible patients.
Conduct facility to home admissions as needed.
Demonstrate excellence in nursing practice and support organizational needs.
