About The Position

The Weekend Hospice RN (On Call) provides nursing care for patients of all ages in their place of residence during weekends. This role involves responding to urgent patient and family needs, managing symptom control, and offering support during end-of-life care. The position requires timely responses, effective clinical assessment, and the ability to work independently. The nurse will also perform emergency admissions of new hospice patients as needed and apply infection control principles.

Requirements

  • Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
  • Current driver’s license.
  • Experience as a registered nurse in a clinical care setting.
  • Must read, write, and comprehend English.
  • Reliable transportation and valid auto liability insurance.

Nice To Haves

  • Hospice experience preferred.
  • Specialized skills preferred in palliation and end-of-life.

Responsibilities

  • Administer nursing care for patients of all ages in their place of residence after hours and/or on weekends.
  • Respond to urgent patient and family needs.
  • Manage symptom control and provide support during end-of-life care.
  • Perform patient assessments, manage symptoms, and administer medications as needed.
  • Collaborate with the interdisciplinary team (physicians, social workers, chaplains, hospice aides, etc.) to ensure comprehensive care.
  • Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Maintain accurate and timely documentation of care provided using EMR.
  • Communicate changes in patient condition to the care team and recommend adjustments to the care plan.
  • Provide emotional support and guidance to patients, families, and caregivers.
  • Ensure that all interventions adhere to hospice philosophy, policies, and regulatory guidelines.
  • Maintain timely response to on-call duties, ensuring patient and family needs are met in a timely manner.
  • Triage duties may be required for some employees in this role, depending on specific assignments and departmental needs.
  • Apply concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Inform physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.
  • Demonstrate commitment and professional growth and competency by participating in the Hospice sponsored in-service programs and maintaining or improving competency.
  • Promote the Hospice’s philosophy and administrative policies.
  • Provide effective communication to patients, family, team members, and other health care professionals.
  • Perform emergency admissions of new hospice patients as needed.
