About The Position

This is a full-time, field-based clinical role for a Hospice Registered Nurse responsible for weekend coverage. The position requires daily travel between patient locations, independent clinical decision-making, and building relationships with patients and families in their own environments. The role offers the opportunity to practice nursing in a personal and impactful setting, providing care to patients wherever they call home, including private residences, assisted living communities, and skilled nursing facilities. The nurse will conduct scheduled and PRN hospice visits, assess patient needs, provide skilled nursing care, monitor conditions, and deliver compassionate support. Additionally, the role involves clinical leadership, including assessing patients, developing care plans, conducting admissions, analyzing clinical information, and initiating crisis care. The Weekend Float RN Case Manager will also provide coverage for RN Case Managers during PTO, leaves of absence, and periods of increased census, functioning as the RN Case Manager when assigned. This includes coordinating disciplines, delegating and supervising LPNs and CNAs, and collaborating with physicians, social workers, chaplains, hospice aides, and community partners. Responsibilities also include coordinating medical equipment, medications, supplies, and contracted services, participating in quality improvement initiatives, and assisting with onboarding and mentoring new employees.

Requirements

  • Current Registered Nurse (RN) license in North Carolina
  • Minimum two years of recent clinical nursing experience
  • Expert understanding of medical terminology and clinical data interpretation
  • Current CPR certification
  • Reliable transportation
  • Strong assessment, documentation, and care planning skills
  • Ability to travel throughout the service area and provide care in homes and facilities
  • Ability to lift and/or carry up to 50 pounds
  • Ability to independently locate resources and collaborate across disciplines
  • Strong communication and patient education skills
  • Competent computer and technology skills

Responsibilities

  • Deliver Expert Hospice Nursing Care
  • Conduct scheduled and PRN hospice visits throughout the community
  • Assess patient needs and provide skilled nursing care aligned with the hospice plan of care
  • Monitor patient conditions and implement appropriate interventions to maximize comfort and quality of life
  • Deliver compassionate support to patients and families navigating serious illness and end-of-life care
  • Provide Clinical Leadership
  • Utilize advanced clinical judgment to assess patients and develop individualized care plans
  • Conduct elective, urgent, and emergent admissions
  • Analyze complex clinical information to determine appropriate levels of care and interventions
  • Initiate Crisis Care in accordance with agency protocols and Medicare regulations
  • Serve as a Flexible Care Team Leader
  • Provide coverage for RN Case Managers during PTO, leaves of absence, and periods of increased census
  • Function as the RN Case Manager when assigned

  • Delegate and supervise LPNs and Certified Nursing Assistants in accordance with regulatory requirements
