About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate Weekend Baylor RN Registered Nurse - Home Health to work with our Home Health team to deliver individualized care to patients in their homes throughout North Hillsborough & East Pasco counties (North Tampa, Dade City, Land O'Lakes, and surrounding areas). The organization provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. This role involves home health visits in North Hillsborough & East Pasco counties.

Requirements

Graduate of an approved nursing program with an active Florida RN license
Minimum of 1 year of direct patient care experience
Willingness to participate in weekend and holiday on-call rotations

Nice To Haves

BSN or MSN degree
Prior home health RN experience strongly preferred
Experience using Homecare Homebase (HCHB) EMR
Strong assessment, critical thinking, and patient education skills

Responsibilities

Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care
