Requirements

  • Current and unrestricted CA RN license
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
  • High level of clinical knowledge in home health care is highly preferred
  • Experience with high acuity patients is a plus
  • Experience working with an interdisciplinary team is highly desired
  • Case Management fundamentals is highly desired
  • Previous experience with IV Medication infusions highly preferred
  • Proficiencies initiating intravenous (IV) therapy is a plus

Responsibilities

  • Conduct the initial assessment of patients to determine home care needs.
  • Conduct Start of Care (SOC) Admissions - OASIS.
  • Conduct follow-ups and evaluation visits as needed in the designated territory.
  • Make home visits to patients in the designated territory.
  • Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures.
  • Monitor clients’ conditions, reporting changes to the Clinical team.
  • Collaborate with physicians and agency staff to deliver the best care possible.
  • Initiate and administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
