About The Position

The Waiver Capacity Coordinator is a professional position within the Office of Developmental Supports (ODS), Allegheny County Department of Human Services (DHS). This position is responsible for the preparation and implementation of data related to the Multi-Year Program Growth Strategy, discovery of resources and administrative functions related to Individual Support Plan (ISP) management for Medically Complex Condition Individuals, as well as maintaining contacts with providers and Supports Coordination Organizations. The Waiver Capacity Coordinator will develop and maintain quality measures for the waiver capacity management team to ensure the WCM team’s vision aligns with DHS and ODP.

Requirements

  • Preparation and implementation of data related to the Multi-Year Program Growth Strategy.
  • Discovery of resources and administrative functions related to Individual Support Plan (ISP) management for Medically Complex Condition Individuals.
  • Maintaining contacts with providers and Supports Coordination Organizations.
  • Develop and maintain quality measures for the waiver capacity management team.
  • Expertise in Medically Complex Conditions waiver services and supports.
  • Support to provider agencies and Supports Coordination Organizations for MCC individuals.
  • Management of ISP Alert Assignments.
  • Maintenance of accurate records of transfers, additions, and removals to the database for individuals being followed by the Planning Team.
  • Accurate tracking of changes to individuals enrolled in each waiver to ensure all waiver capacity is accounted for.

Responsibilities

  • Within the Multi-Year Program Growth Strategy, develop and implement a plan to end the waiting list for individuals over the age of 21 with ID/A with Emergency need in Allegheny County.
  • Develop and maintain accurate data related to Multi-Year Program Growth Strategy, including but not limited to utilization of current waivers and budget changes for individuals enrolled in waivers and those that have vacated waivers.
  • Complete monthly and quarterly reviews for data related to Multi-Year Program Growth Strategy.
  • Facilitate internal planning meetings for Multi-Year Program Growth Strategy.
  • Reviewing utilization of waiver services for individuals including new to waiver.
  • Leading analytics and documentation reporting for QA&I.
  • Reviewing, changing and updating information relating to the closure of a waiver.
  • Investigate reasoning a record did not close appropriately and correct.
  • Schedule and prepare agenda for quarterly meetings with Supports Coordination Organizations.
  • Create and maintain contacts with Medically Complex Condition community service resources.
  • Become an expert in Medically Complex Conditions waiver services and supports.
  • Provide support to provider agencies and Supports Coordination Organizations for MCC individuals.
  • Manage the waitlist for Medically Complex Conditions.
  • Management of ISP Alert Assignments.
  • Maintain accurate records of transfers, additions, and removals to the database for individuals being followed by the Planning Team.
  • Accurate tracking of changes to individuals enrolled in each waiver to ensure all waiver capacity is accounted for.
  • All other duties as assigned by supervisor.
