Assertive Community Treatment (ACT) is a comprehensive community-based treatment program for persons with severe and persistent mental illness. The ACT Team is a multi-disciplinary group of mental health professionals organized to help provide treatment, rehabilitation, and support services to consumers in the community.

Responsibilities

  • Carrying out rehabilitation and support functions with the consumers.
  • Assisting in treatment through implementing substance abuse services, providing education, support, and consultation to families, and providing crisis intervention.
  • Communicating with consumers on a weekly basis to determine progress and revise the service plan as necessary.
  • Assisting consumers with resolution of problems and/or concerns.
  • Developing and enhancing consumer network of vocational supports with their families in the community, neighborhood, and workplace settings.
  • Observing and responding to significant changes in consumer mental and physical health and communicating changes to the consumer's treatment team.
  • Collaborating with county Office of Mental Health/Intellectual Disability personnel and other service providers involved in the consumer's treatment to assess consumer needs and provide the most appropriate services.
  • Providing 24 hour a day, 7 day per week on-call crisis services on a rotational basis.
  • Maintaining case records and completing appropriate documentation, letters, assessments and summaries as needed or designated by established policies and procedures.
  • Understanding and following company policies; encouraging others to follow work rules; calling attention to actions that may violate policies and procedures.
  • Complying with basic job requirements and tasks; arriving at work on time as scheduled; proactively informing others when he/she encounters problems that may limit his/her ability to meet expectations.
  • Developing treatment plans and crisis plans based on the client’s strengths and needs.
  • Gaining an understanding of the client’s relevant personal, familial, social, vocational, and cultural data.
  • Preparing and completing all necessary documentation, including assessments, new consumer admissions, medication records, progress notes, incident reports, treatment and crisis plans and miscellaneous paperwork/reports within required timeframes as requested.
  • Assisting the client in gaining funding authorization for needed services and entitlements.
  • Participating in agency investigations.
  • Providing individual vocational supportive counseling, and establishing vocational and career goals, and plans that recognize symptoms of mental illness that interfere with work.
  • Planning and providing work related supportive services including development of job seeking skills, development of jobs based on client abilities and interest, job coaching and support on both on and off the job site.
  • Developing and maintaining relationships with employers in the community setting.
  • Provided supervision and guidance from supervisor, by attending daily team meetings and individual supervision.
