Bayshore is currently seeking a passionate and caring Visiting Nurse to join their exceptional care team in the community. This role involves delivering personalized, high-quality care to clients in their community, acting as a primary care planner with the autonomy and flexibility to provide a holistic approach to care. The role also involves teaching, delegating tasks, and maintaining the highest level of practice. The Visiting Nurse will be the nursing care expert for their community, with efficient work schedules designed to maximize care time and minimize travel time.

Requirements

  • Graduate of an accredited School of Nursing (RPN)
  • Current registration in good standing with the provincial regulatory body (CNO)
  • 2-5 years of recent relevant nursing experience in a fast-paced clinic or medical environment
  • Valid G2 or G Driver’s License and access to a reliable vehicle
  • Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions
  • Must be able to travel from home-to-home visiting clients within the Greater Hamilton/Burlington areas in all weather conditions

Responsibilities

  • Participate in on-call rotations as required (typically twice per month overnights)
  • Conduct in-home assessments and deliver nursing care based on individualized care plans
  • Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
  • Monitor patient progress and update care plans accordingly
  • Educate patients and caregivers on disease management, medication adherence, and health promotion
  • Collaborate with interdisciplinary teams and task delegation to ensure continuity of care
  • Maintain accurate and timely documentation using electronic health records
