Bayshore is currently seeking a passionate and caring Visiting Nurse to join their exceptional care team in the community. This role involves home-to-home visits in the Greater Hamilton and Burlington Area, requiring driving within the community. The position offers pay per visit with shifts from Monday through Friday, alternating weekend rotations, and on-call and statutory holiday rotations. Shift times vary and are assigned. As a Visiting Nurse, you will play a hands-on role delivering personalized, high-quality care to clients in their community. You will act as a primary care planner, providing a holistic approach to care and increasing your skills through comprehensive training, mentorship, and preceptorship programs. You will also teach and delegate tasks to clients and family members while maintaining the highest level of practice. The role emphasizes efficient work schedules to focus more on care time and less on travel time.

Requirements

Graduate of an accredited School of Nursing (RPN)
Current registration in good standing with the provincial regulatory body (CNO)
2-5 years of recent relevant nursing experience in a fast-paced clinic or medical environment
Valid G2 or G Driver’s License and access to a reliable vehicle
Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions
Must be able to travel from home-to-home visiting clients within the Greater Hamilton/Burlington areas in all weather conditions

Experience with wound care, diabetes management, trach/vent, IV care

Responsibilities

Participate in on-call rotations as required (typically twice per month overnights)
Conduct in-home assessments and deliver nursing care based on individualized care plans
Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
Monitor patient progress and update care plans accordingly
Educate patients and caregivers on disease management, medication adherence, and health promotion
Collaborate with interdisciplinary teams and task delegation to ensure continuity of care
Maintain accurate and timely documentation using electronic health records
