The Visit Nurse (RN) is responsible for conducting and documenting thorough nursing assessments, identifying variations from baseline patient status, and evaluating care, treatment, and services. This role involves identifying patient and family educational needs, utilizing knowledge of Hosparus Health Patient Care Guidelines and Clinical Protocols, and collaborating with physicians regarding care and treatment. The nurse develops and revises the Plan of Care, initiates and revises CNA plans, and supervises CNA care. Key responsibilities include demonstrating clinical competency in symptom and pain control, palliative care, preparing patients and caregivers for discharge and the dying process, and evaluating medical eligibility criteria. The role also requires clear communication with team members, coordination of care with support staff and interdisciplinary teams, and effective problem-solving. The nurse will communicate with community partners and referral sources, participate in on-call rotations, and work towards effective, cost-efficient care. Responsibilities include assuming responsibility for preauthorization of external services, completing medication reconciliation, collaborating with management on duties and coverage, and timely point-of-service documentation. The nurse must achieve organizational standards for visits per week, visit length, and documentation timeliness. Additionally, the role requires knowledge of hospice admission criteria, the ability to communicate the hospice philosophy, and conducting admission visits to assess patient needs and medical eligibility. The Visit RN serves as a professional role model and may act as a nurse preceptor.
