About The Position

Baba is rebuilding healthcare for older adults by providing expert advocates who coordinate care. This role involves performing brief, structured telehealth visits to establish medical necessity for care navigation programs. These intake visits are crucial for identifying high-risk conditions, unmet social needs, and confirming eligibility for ongoing care navigation services. The Nurse Practitioner will collaborate with care advocates and the clinical operations team to ensure safe, compliant, and compassionate patient enrollment into the appropriate level of support. This is a remote, 1099 (contractor) position, ideal for physicians seeking flexible, high-impact telehealth work.

Requirements

• Completion of an accredited Nurse Practitioner program.
• Active Nurse Practitioner or APRN license, in good standing - licensed in multiple states.
• 2+ years of experience in primary care, telehealth, or care management.
• Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation.
• Proficiency with EMRs, care management platforms, and telehealth workflows.
• Bilingual Spanish required.

Responsibilities

• Perform telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity for care navigation services and identify high-risk conditions or unmet social needs.
• Document findings in Baba’s platform, ensuring accurate coding and use of SDOH Z-codes, diagnoses, and risk factors.
• Develop and approve individualized care plans to ensure alignment with the patient’s medical treatment plan and documentation of continued medical necessity.
• Collaborate with advocates and nurse care managers to finalize individualized care plans aligned with each patient’s medical and social needs.
