About The Position

Requirements

Board Certified as a Family Nurse Practitioner
Active Family Nurse Practitioner license in Louisiana
Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
Strong written and verbal communication skills
Ability to work independently or in a team environment
Pass national background check and valid clinical license search

Responsibilities

Conduct virtual wellness visits, including adult and/or pediatric wellness visits and health assessments.
Perform thorough health risk assessments, medication reviews, and cognitive screenings.
Educate patients on their health and well-being.
Collaborate with in-home clinical team members (Sprinters) to evaluate vital signs and arrange blood draws.
Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, and community resources.
Document findings accurately.
Review case load and prepare technology at the beginning of each day.
Navigate daily schedule using modern technology.
Make connections with diverse patients and collect relevant information.
