About The Position

Sprinter Health is seeking dynamic Nurse Practitioners for a remote Virtual Family Practice role. This position involves conducting virtual wellness visits for patients in their homes, leveraging medical expertise to offer services such as virtual adult and/or pediatric wellness visits and health assessments. The ideal candidate will have prior experience with wellness visits, possess traits like dependability, professionalism, and problem-solving abilities, and be committed to exceptional customer service and autonomous work. A friendly, compassionate, and empathetic approach to personalized patient care is essential.

Requirements

  • Board Certified as a Family Nurse Practitioner
  • Active Family Nurse Practitioner license in Louisiana
  • Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
  • Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
  • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
  • Strong written and verbal communication skills
  • Ability to work independently or in a team environment
  • Pass national background check and valid clinical license search

Responsibilities

  • Conduct virtual wellness visits, including adult and/or pediatric wellness visits and health assessments.
  • Perform thorough health risk assessments, medication reviews, and cognitive screenings.
  • Educate patients on their health and well-being.
  • Collaborate with in-home clinical team members (Sprinters) to evaluate vital signs and arrange blood draws.
  • Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, and community resources.
  • Document findings accurately.
  • Review case load and prepare technology at the beginning of each day.
  • Navigate daily schedule using modern technology.
  • Make connections with diverse patients and collect relevant information.
