About The Position

The Virtual Care RN is responsible for providing and coordinating high-quality patient care to various patient populations within the framework of the Advanced Hospital at Home mode. The nursing care is provided from a centralized location, supported by technology and service provider network. The RN assesses the patient utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home. They navigate patients and families across the continuum of the Advanced Care at Home phases of care, monitor and respond to biometric data, and conduct home and audio/visual patient care visits as directed by the care team and/or guidelines.

Requirements

Diploma in Nursing required or Associate degree from an accredited Nursing program required
3 years RN experience required; 1 year in the last 3 years as a Registered Nurse in specialty area of acute care and patient population of position ( e.g. adults, geriatrcs)
RN - Registered Nurse upon hire required
CPR within 30 days of hire (maintained every 2 years)
Able to promote patient safety and a positive patient experience
Flexible and comfortable with a changing and dynamic work environment and schedule across all shifts and days
Team player who also works well independently
Strong interpersonal, inter-professional communication and collaboration capabilities
Ability to analyze, interpret and problem solve quickly with well-articulated action planning
Strong verbal/written communication and conflict resolution skills
Strong empathy to deliver compassionate, patient centered care
Communicates effectively in interprofessional teams including peers, nursing, ancillary professionals and supplier providers
Has professional and respectful interactions with patients, caregivers, and members of the interprofessional team
Clinical knowledge
Ability to use technology in addition to Epic

Nice To Haves

BSN preferred
8 years of acute care nursing experience is strongly preferred
Certification in specialty fields such as acute care, oncology, or geriatrics is desirable.
Critical Care or Med/surg experience preferred

Responsibilities

Participates in ongoing learning and continuing education to care for a variety of patient populations.

Supports the transitioning of patients effectively within and across health delivery systems.
