About The Position

Concierge Home Care is redefining patient care through innovation and compassion. This is an opportunity for an experienced LPN with home health and triage experience to make a real difference in a remote setting. The schedule is every weekend, Friday, Saturday & Sunday, from 8:00 AM to 8:00 PM (Full 12-hour shifts). As a Virtual Care Nurse (LPN), you’ll support high-risk patients and help prevent hospitalizations through proactive, remote care.

Requirements

• Active, unencumbered Florida LPN license (required)
• 2+ years of home health experience (required)
• Strong background in triage & hospitalization prevention
• Comfortable with telehealth technology & documentation systems
• Employment may require successful completion of an AHCA Level 2 background screening.

Nice To Haves

• Experience with high-risk or complex patients preferred
• Proficiency in Microsoft Office (preferred)

Responsibilities

• Conduct virtual patient visits and clinical assessments
• Provide education on chronic disease management
• Monitor telehealth data and identify risk trends
• Perform triage and early intervention to prevent rehospitalization
• Collaborate with interdisciplinary care teams
• Participate in care strategy meetings and performance improvement initiatives
• Support development of virtual care best practices
