About The Position

knownwell is a weight-inclusive healthcare company providing metabolic health services, primary care, nutrition counseling, and behavioral health services. Its hybrid care model combines in-clinic and virtual services. The Virtual Care Coordinator supports patients and clinicians nationwide by coordinating care delivered by the Interdisciplinary Care Team, including Health Coaching and Registered Dietitians. The role triages and resolves non-clinical patient needs and helps ensure a seamless experience in a fully remote environment. It reports to the Manager of Virtual Care Operations and is suited to someone with virtual care experience who can navigate EMRs, manage competing priorities, and work in a fast-paced, digital-first health tech startup.

Requirements

• High school diploma or equivalent.
• At least 2 years of experience in virtual care, patient coordination, or a Medical Assistant role.
• Familiarity with virtual care workflows in a primary care or chronic care setting.
• Prior experience supporting care in a multi-state, remote care delivery model.
• Strong communication and organizational skills.
• Comfort using remote tools, secure messaging, and EMR platforms.
• Tech-savvy, adaptable, and comfortable working independently in a remote setting.

Nice To Haves

• Experience with Athena EMR is preferred.

Responsibilities

• Partner with cross-functional teams, including Call Center, Patient Access, and Billing, to coordinate patients’ health coaching and/or nutrition journeys.
• Contact patients to help them navigate their care, including insurance and payment considerations, service options, and appointment scheduling.
• Manage inbound patient messages and escalate clinical questions and concerns to the appropriate clinician.
• Lead case-by-case service recovery, including developing and executing action plans and coordinating patient communications to resolve concerns.
• Manage Remote Patient Monitoring (RPM) program enrollment workflows and device logistics with internal teams and external vendors.
• Provide real-time administrative support and issue resolution to clinicians and patients during virtual visits, including technical troubleshooting as needed.
• Collaborate with the Manager of Virtual Care Operations and cross-functional team members to identify workflow and operational process improvements.
• Deliver a warm, inclusive, and professional patient experience aligned with knownwell’s values.
