About The Position

Navitas Healthcare, LLC is seeking a Virtual Care Coordinator for a contract position in New York, NY. This role is crucial for supporting telehealth programs by performing quality assurance, peer reviews, and trend analysis. The coordinator will be responsible for conducting monthly meetings with leadership to discuss virtual care metrics and workflows, as well as triaging and assigning virtual monitoring cases and managing patient eligibility tracking. A key aspect of the role involves monitoring real-time patient data to identify clinical changes, safety risks, and escalations. The coordinator will also provide support for telehealth platforms, troubleshooting system or workflow issues, and documenting clinical observations, interventions, and operational reports. Training staff on telehealth workflows, EMR systems, and documentation standards is also a responsibility. Collaboration with clinical teams, IT partners, and vendors is essential for workflow improvements. The position requires ensuring HIPAA compliance and patient privacy during virtual care delivery, and submitting shift reports to maintain operational efficiency.

Requirements

  • Minimum 3 years direct patient care experience required
  • 2+ years telehealth or remote monitoring experience required
  • Experience with EMR systems
  • Strong analytical, communication, and multitasking skills
  • Ability to identify clinical risks and escalate appropriately
  • Familiarity with telehealth workflows and virtual care systems

Nice To Haves

  • Associate’s or Bachelor’s degree in a health-related field
  • Epic EMR system experience
