About The Position

The Veterans Case Manager is a key member of the clinical team, dedicated to supporting military veterans focusing on transition toward long-term recovery and integration into the community. This position serves the military veterans aiming for recovery, reintegration into the community, and independent living. Through addressing social determinants of health and removing structural obstacles, the Case Manager facilitates successful client outcomes. Key responsibilities include running therapeutic groups, coordinating internal and external client care, and utilizing a closed-loop referral system to ensure seamless service delivery. As part of Axiom Care’s commitment to Culturally and Linguistically Appropriate Services (CLAS), this position supports efforts to provide inclusive and accessible translation services for clients. Responsibilities may include participating in CLAS-related training, supporting language access initiatives, and promoting cultural sensitivity in day-to-day operations. Founded in 2012, Axiom Care is a Phoenix-based behavioral health organization that aims to ensure every Arizonan has access to compassionate, effective substance use treatment. With facilities in Maricopa and Pinal Counties, we offer comprehensive services including 24/7 observation, inpatient detoxification, residential treatment, outpatient treatment, and recovery housing with a focus on underserved populations. Beyond our full-continuum community programs, we proudly operate specialty programs for the justice-involved (in partnership with ADCRR), for members of the Navajo Nation (in partnership with the Navajo Department of Health), and for Veterans (co-located on the Victory Place campus). Requirements

  • Must be proficient in computer applications and electronic medical record systems.
  • Proficient with Microsoft Office Suite or related software.
  • Some assignments require an Arizona Fingerprint Clearance Card.
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Valid Driver’s License with clean driving record, as determined by BBC’s insurance carrier.
  • Ability to operate 15-passenger van.
  • Must be 25 years of age (requirement in order to drive company vans).
  • Knowledge of Windows, basic email, Microsoft Word, timekeeping software, and basic computer navigational skills.
  • Ability to write professional correspondence and reports free from grammatical and spelling mistakes.
  • Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

  • High level of patience.
  • Active CPR and First Aid certification required (may be provided by Axiom).
  • Active Fingerprint Clearance Card (residential locations).
  • Negative TB Test result within past 12 months (residential locations).
  • Minimum one year of case management knowledge and experience with the AHCCCS population, justice involved persons, chronically underhoused and the substance use treatment.
  • Demonstrating an understanding of basic clinical concepts and practices to support client needs and team objectives (Entry-Level Clinical Knowledge).
  • Uses appropriate interpersonal styles and techniques to reduce tension and/or conflict between two or more people; able to size up situations quickly; able to identify common interests; facilitates resolution (De-escalation).
  • Identifying and connecting clients to appropriate services, including SMI evaluations (Clinical Evaluation).
  • Demonstrates the ability to adapt approach based on client's various levels of trauma. Makes clients and their needs a primary focus of one’s actions; gains client trust and respect; meets or exceeds client’s expectations (Trauma Informed Client Care).
  • Organizes work, sets priorities, and determines resources requirements; determines necessary sequence of activities needed to achieve goals (Planning and Organizing).
  • Adapts well to changes in assignments and priorities; adapts behavior or work methods in response to new information, changing conditions, or unexpected obstacles; approaches change positively and adjusts behaviors accordingly (Adaptability).
  • The ability to function effectively as a member of an inter-professional team that includes behavioral health and primary care providers, and clients. Participates as an active and contributing member of a team to achieve team goals. Works cooperatively with other team members, involves others, shares information as appropriate, and shares credit for team accomplishments (Collaboration & Teamwork).
  • Knowledge of regulations. Prepare accurate, concise notes. Write comprehensive, clear psychosocial narrative. Record client progress in relation to treatment goals. Appropriate verbiage. Discharge summaries (Documentation).

Nice To Haves

  • Veteran of the U.S. Armed Forces (honorable discharge preferred).
  • Personal or professional experience in recovery or substance use treatment.
  • Familiarity with 12-Step programs, VA systems, or trauma-informed care.
  • One year of experience with the Crisis/SMI population strongly preferred.

Responsibilities

  • Facilitate and document group sessions, focusing on recovery, life skills, and other relevant topics.
  • Utilize evidence-based practices to foster engagement and progress in group settings.
  • Develop and implement client-driven treatment plans tailored to individual needs.
  • Coordinate care internally with clinical and housing staff, ensuring alignment with treatment goals.
  • Liaise with external providers and community resources to arrange necessary services, maintaining communication to close the referral loop.
  • Carry a caseload of clients, guiding them through all program phases.
  • Assist clients with employment services.
  • Provide education and connect clients with community resources tailored to their needs.
  • Maintain accurate and timely electronic medical records, documenting activities, interventions, client contact, staffing notes, and treatment planning.
  • Serve as the primary point of contact between the treatment team and external providers.
  • Attend and contribute to site meetings, staff training sessions, and clinical oversight.
  • Transport clients to appointments as needed.
  • Identify and report ongoing maintenance and safety issues at the facility.
  • Perform other duties as assigned by management, aligned with the organization’s mission and goals.
  • Participating in CLAS-related training, supporting language access initiatives, and promoting cultural sensitivity in day-to-day operations.
