About The Position

The Veterans Case Manager is a key member of the clinical team at Axiom Care, dedicated to supporting military veterans in their transition toward long-term recovery and integration into the community. This position focuses on helping veterans achieve recovery, reintegration, and independent living by addressing social determinants of health and removing structural obstacles. The Case Manager facilitates successful client outcomes through running therapeutic groups, coordinating internal and external client care, and utilizing a closed-loop referral system to ensure seamless service delivery. Axiom Care, founded in 2012, is a Phoenix-based behavioral health organization providing comprehensive substance use treatment services across Maricopa and Pinal Counties, including 24/7 observation, inpatient detoxification, residential treatment, outpatient treatment, and recovery housing, with a focus on underserved populations and specialty programs for justice-involved individuals, Navajo Nation members, and Veterans.

Requirements

  • Must be proficient in computer applications and electronic medical record systems.
  • Proficient with Microsoft Office Suite or related software.
  • Some assignments require an Arizona Fingerprint Clearance Card.
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Valid Driver’s License with clean driving record, as determined by BBC’s insurance carrier.
  • Ability to operate 15-passenger van.
  • Must be 25 years of age (requirement in order to drive company vans).
  • Knowledge of Windows, basic email, Microsoft Word, timekeeping software, and basic computer navigational skills.
  • Ability to write professional correspondence and reports free from grammatical and spelling mistakes.
  • Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

  • Active CPR and First Aid certification required (may be provided by Axiom).
  • Active Fingerprint Clearance Card (residential locations).
  • Negative TB Test result within past 12 months (residential locations).
  • Minimum one year of case management knowledge and experience with the AHCCCS population, justice involved persons, chronically underhoused and the substance use treatment.

Nice To Haves

  • A strong knowledge and understanding of veterans/military population.
  • Veteran of the U.S. Armed Forces (honorable discharge preferred).
  • Personal or professional experience in recovery or substance use treatment.
  • Familiarity with 12-Step programs, VA systems, or trauma-informed care.
  • CPR and First Aid certification (can be provided by the organization).
  • One year of experience with the Crisis/SMI population strongly preferred.

Responsibilities

  • Facilitate and document group sessions, focusing on recovery, life skills, and other relevant topics.
  • Utilize evidence-based practices to foster engagement and progress in group settings.
  • Develop and implement client-driven treatment plans tailored to individual needs.
  • Coordinate care internally with clinical and housing staff, ensuring alignment with treatment goals.
  • Liaise with external providers and community resources to arrange necessary services, maintaining communication to close the referral loop.
  • Carry a caseload of clients, guiding them through all program phases.
  • Assist clients with employment services.
  • Provide education and connect clients with community resources tailored to their needs.
  • Maintain accurate and timely electronic medical records, documenting activities, interventions, client contact, staffing notes, and treatment planning.
  • Serve as the primary point of contact between the treatment team and external providers.
  • Attend and contribute to site meetings, staff training sessions, and clinical oversight.
  • Transport clients to appointments as needed.
  • Identify and report ongoing maintenance and safety issues at the facility.
  • Perform other duties as assigned by management, aligned with the organization’s mission and goals.
  • Support efforts to provide inclusive and accessible translation services for clients, including participating in CLAS-related training, supporting language access initiatives, and promoting cultural sensitivity in day-to-day operations.
