The Case Manager is responsible for providing comprehensive case management and coordinated supportive services to eligible veterans, surviving spouses, and dependent family members seeking assistance through the Veteran and Family Relief General Assistance Program funded by the Texas Veterans Commission. The Case Manager conducts client assessments, determines program eligibility, develops individualized service plans, and provides emergency financial assistance for eligible rental, utility, and other approved basic needs in accordance with grant requirements. The Case Manager works collaboratively with participants to promote eviction prevention, housing stability and long-term self-sufficiency by connecting them with employment services, workforce development, financial capability education, benefits enrollment, and other community resources. Additional responsibilities include client outreach and engagement, comprehensive case management, documentation of eligibility and services, maintenance of grant-compliant client files, data entry, performance and outcome tracking, monthly reporting, and ensuring compliance with all Texas Veterans Commission, agency, and funding requirements. The Case Manager is expected to maintain strong partnerships with veteran-serving organizations and community providers while delivering services with professionalism, compassion, and a commitment to helping veterans and their families achieve lasting stability.
