About The Position

CenterWell in The Villages, a subsidiary of Humana Inc., is looking for a primary care physician for our Creekside clinic in The Villages, FL. This role offers a unique care model focused on senior-primary care with a versatile approach. The physician will have the opportunity to take a hands-on approach, addressing patient needs inside the exam room and looking beyond to understand how social determinants of health are affecting health outcomes. The physician will report to the Associate Medical Director and the role is onsite and in-person at the Creekside Clinic in The Villages, FL. The Primary Care Physician will focus on outpatient medicine, continuity of care, health maintenance, and disease prevention. Responsibilities include keeping medical history and records, referring patients to specialists, and influencing clinic department strategy. The physician will make decisions on moderately complex to complex issues and will have considerable latitude in determining best approaches to assignments.

Requirements

• Doctor of Medicine or Osteopathic Medicine
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license
• This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Nice To Haves

• Minimum of two+ years of experience
• 1+ years of experience managing Medicare Advantage panel of patients
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.

Responsibilities

• Focus on outpatient medicine, continuity of care, health maintenance, and disease prevention.
• Keep a medical history and medical records.
• Refer patients to specialists.
• Influence clinic department's strategy.
• Make decisions on moderately complex to complex issues.
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care.
