About The Position

Honeycomb Management is a Managed Services Organization that manages primary care practices and specialty care practices with multiple locations in Memphis, Tennessee and Mississippi. The Value-Based Care Coordinator - LPN - will facilitate and support operations and communication strategies for all Value-Based care agreements. Under the direction of the Quality and Stars Program Manager, the Value-Based Care Coordinator – LPN will coordinate the planning, oversight and completion of projects and complex tasks needed to ensure Honeycomb Medical Group’s successful performance in value-based contracts and compliance. The individual in this role is expected to incorporate continuous process improvement into daily work using critical thinking and problem-solving skills to identify opportunities for improvement while promoting organizational and contract engagement in solution development. The individual in this role must have knowledge of Care Coordination, Preventative Health, and Preventative Screenings.

Requirements

  • Graduate of an accredited Practical Nursing Program.
  • Current Licensed Practical Nurse (LPN) license in practicing state.
  • Must possess and maintain a valid driver’s license required.
  • Must have strong Electronic Medical Record (EMR) skills.
  • Must be able to effectively navigate patient engagement software to conduct recalls.
  • Ability to perform basic Excel skillsets.

Nice To Haves

  • Three (3) years of experience as a Licensed Practical Nurse (LPN) preferred.
  • Experience in HEDIS and STAR Measures are preferred.

Responsibilities

  • Demonstrate knowledge regarding all Value-Based policies and procedures, governance structure and regulatory requirements through ongoing research and regular attendance at training webinars.
  • Provide superior customer service to patients by exemplifying effective communication and interpersonal skills.
  • Utilize Motivational Interviewing to support patient engagement and attention to patient health goals.
  • Must have the ability to meet assigned deadlines and flexibility to adapt to changing priorities.
  • Able to understand complex and chronic care conditions affecting patient health and wellness as they relate to gap closures.
  • Provide clinical support for other site locations as needed.
  • Perform other duties as assigned.
