About The Position
Shanadoa Home Health is hiring a VA Home Care Program Manager (RN) to lead and grow our Veteran home care program in Tulsa and Bartlesville. This role involves leading and supporting the aide team serving Veterans at home, ensuring all visits are authorized and documented correctly, building relationships with VA care teams to expedite Veteran care, and conducting monthly RN visits as the program expands. The agency has been caring for Oklahomans for 19 years and is seeking a dedicated nurse to build a meaningful program.
Requirements
- An Oklahoma RN license
- Home health experience
- Hands-on experience with VA community care
- Current, unrestricted Oklahoma RN license (or compact)
- 3+ years of home health or community-based nursing experience
- Hands-on experience with VA community care referrals and authorizations
- Experience supervising aides or paraprofessional staff
- Valid driver's license, reliable transportation, and auto insurance
Nice To Haves
- Military or Veteran-family background is a plus
- Experience running or coordinating a VA Homemaker/Home Health Aide program
- Established relationships with Oklahoma VA facilities and care teams
- Military, Veteran, or military-family background
Responsibilities
- Lead and support our aide team serving Veterans at home
- Make sure every visit is authorized, documented, and done right
- Build strong relationships with VA care teams and help more Veterans get care faster
- See Veterans for monthly RN visits as the program grows
- Supervise and support home health aides; address clinical concerns in the field
- Review aide documentation and schedules against authorized services
- Monitor authorizations and renewals to prevent lapses in care
- Conduct program audits and lead corrective action
- Complete monthly RN visits for VA clients until a dedicated visiting nurse is hired
- Build relationships with VA community care staff, social workers, and care coordinators
- Streamline the referral-to-admission process and grow Veteran census
