About The Position

Under the direction of the Urgent Care Nurse Manager, and under the Supervision of the Nursing Supervisor. The Registered Nurse (RN) will be responsible to provide, delegate and coordinate nursing care within the registered Nurse scope of practice, and in accordance with San Ysidro Health policies and procedures in an acute ambulatory care setting. The RN should be able to demonstrate the knowledge and skills necessary to provide individualized care in a manner conducive to comfort, safety, and welfare of patients.

Requirements

  • 1 year of acute care experience within the past 3 years.
  • High School diploma or GED.
  • Graduate of an accredited registered nursing program
  • BLS/ACLS and PALS certification from AHA (American Heart Association)
  • Possession of active valid California RN license
  • Ability to read, write and comprehend medical information and terminology.
  • Ability to effectively present information, both verbal and written.
  • Demonstrated ability to use personal computer.
  • Effective interpersonal and communication skills.
  • Demonstrates accountability and professionalism.
  • Basic computer skills and experience or familiarity with EMR.
  • Experienced in NextGen or Epic - electronic health records
  • Knowledge of legal implications in clinical practice and documentation
  • Continuous standing/walking/twisting.
  • Occasionally lifts pushes and/or transfers patients.
  • Exposed to unpleasant elements (accidents, injuries, and illness).
  • Reliable transportation necessary for traveling to assist with providing coverage as needed throughout all SYH clinic locations.
  • Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
  • Compliance with all mandated vaccinations and all boosters is a term and condition of employment.

Responsibilities

  • Provide direct patient care to a diversified patient population, applying knowledge, experience, and judgement in determining importance of situations and setting priorities.
  • Participate in staff education, demonstrate leadership skills, and accountability for own practice as defined by the Nurse Practice Act

  • Accurately identify patient needs based on initial interpretation of observation and clinical knowledge and judgement.
  • Implement patient care functions such as comprehensive triage process, primary assessment to determine acuity level, and initiation of specific interventions or test orders as per SYH nursing protocols when applicable.
  • Perform secondary assessment focusing on medical/surgical history, current medications, social practices, physiologic, psychosocial, cultural, spiritual, and cognitive status.
  • Perform nursing tasks including but not limited to focused assessment based on visit complaint, EKGs, Intravenous (IV) access insertion, prepare and administer IV medications as per provider’s order, urine catheter insertion, respiratory treatments, wound care, and provide patient and or family education upon disposition.
  • Display understanding of standard pharmacological agents used in treating patient conditions.
  • Demonstrate competency and compliance with infection prevention and control measures, sterile techniques, isolation, and universal precautions.
  • Applies principles of aseptic technique and infection control as directed by the Infection Control policy and procedures.
  • Assists the physician with therapeutic and diagnostic patient care functions which may require sterile technique, removal, or application of dressings, suturing or wound care.
  • Performs inhalation treatments, prepares, and administers immunizations and medications (oral and injections) as directed by physician order and in accordance with SYH policy and procedure.
  • Participates and responds, when assigned to clinical emergency codes.
  • Report any treatment results, observations, patient complaints, or any variation from baseline to the provider in a timely manner.
  • Answer urgent call bell promptly; assist patients if within own scope of practice or report the request/complaint to appropriate personnel.
  • Document all pertinent patient information (i.e., completed orders, nursing procedures, patient’s response to treatment, etc.) in the EMR, as established by SYH guidelines.
  • Monitor patient’s response to treatment and communicate with other health team members significant changes and modify care plan accordingly.
  • Coordinate hospital admissions or transfer to higher level care facility, in accordance with CMS, SYH, or receiving facility regulations.
  • Demonstrate understanding of policies, protocols, organizational structure, and appropriately utilize chain of command.
  • Participates regularly in staff development trainings to ensure continuous quality improvements.
  • Demonstrate and apply knowledge of safety principles as identified by SYH.
  • Always maintain department readiness (i.e., clean/organized work area, restocking, equipment checks, and exam room preparation)
  • Fulfill Nurse assign responsibilities as assigned on a rotation schedule, or as needed.
  • Responsible for Monthly QA on all clinic equipment
  • Perform other related duties as necessary.
  • Practices customer service standards as defined by SYH and specified clinic.
  • Anticipates the customer's needs, takes proactive steps to meet those needs, by listening and taking responsibility within scope to ensure issues are resolved.
