Group Health Cooperative of South Central Wisconsin is seeking a Care Team Specialist (CTS) to join their collaborative, patient-centered clinic team. This role is crucial for supporting both clinical and administrative workflows to ensure high-quality care and a smooth patient experience. The CTS Urgent Care participates as a member of a care team and supports high quality patient care, including engaging patients in maintaining their physical and mental well-being. They perform technical, clinical, and administrative tasks that contribute to the smooth workflow and quality patient care experience in the clinic. Responsibilities include pre-visit preparation, monitoring patient flow, rooming patients, obtaining patient histories and vital signs, managing certain test results following protocols, supporting Epic in-basket workflow, returning phone calls, and assisting with medical examinations and procedures. The CTS Urgent Care also assists and collaborates with Registered Nurses and Medical Providers in patient care management and coordination, and participates in the review of team/provider in-baskets and My Chart messages. Practitioner and Registered Nurse staff may delegate assignments to the Care Team Specialist UC.
