About The Position

CareOne Lowell is seeking an RN Unit Manager for their Long Term Care, Short Term Rehab, and Skilled Nursing units. This role is responsible for the day-to-day coordination and oversight of all aspects of an assigned clinical unit, ensuring high-quality resident care and regulatory compliance. Requirements

  • Must be an actively licensed RN in assigned state(s) of employment.
  • Previous supervisory experience.
  • Two years of LTC experience.

Responsibilities

  • Ensure required staffing levels are met for each shift daily to provide care for assigned residents.
  • Make staffing level adjustments as needed each shift and update the posted schedule accordingly.
  • Attend, participate, and lead the weekly resident "At Risk" meeting to identify and address resident care concerns.
  • Attend and participate in morning clinical review meetings, PPS meetings, and monthly QA and A meetings.
  • Monitor the provision of resident care by licensed nurses and CNAs, ensuring required documentation is completed.
  • Review 24-hour resident change in condition reports daily, ensuring necessary follow-up, interventions, and notifications occur.
  • Review resident medical records and documentation to ensure clinical programs are in place per policy and procedures.
  • Identify staff educational needs and collaborate with the Facility Educator to meet them.
  • Participate on other committees as requested by the Administrator/Director of Nursing Services.
  • Ensure resident care and documentation comply with state and federal rules and regulations.
  • Comply with and ensure staff compliance with HIPAA rules and regulations, reporting any violations.
  • Make work assignments for assigned staff, including breaks and lunches, and ensure compliance.
  • Initiate disciplinary action as necessary for staff members in collaboration with DON/ADON.
  • Communicate with the appropriate department manager/DON and participate in resolving grievances/concerns from personnel, residents, or family members.
  • Ensure physician orders are being followed.
  • Assist the DON/ADON with preparing performance evaluations.
  • Greet new admissions and families, and assist nursing staff with the admission process.
  • Ensure all admission documentation, including clinical assessments and care plans, is complete within 24-48 hours.
  • Monitor medication passes and treatment schedules to ensure accuracy and timeliness.
  • Provide direct nursing care as necessary.
  • Report problem areas to the Director of Nursing and assist in developing and implementing corrective action.
  • Attend and participate in continuing education programs to stay current with nursing practice standards and maintain licensure.
  • Ensure nursing staff follows policy and procedures, including dress code.
  • Ensure nursing staff participate in fire safety and disaster preparedness drills.
  • Ensure nursing personnel follow established infection control procedures, including isolation precautions.
  • Ensure all personnel operate nursing equipment in a safe manner.
  • Participate in the development of resident care plans, identifying needs, interventions, and responsible services.
  • Ensure all nursing care is provided in privacy and staff knock before entering resident rooms.
  • Report and assist with the investigation of any allegations of resident abuse or misappropriation of property.
  • Assist Social Services with ensuring Advance Directive/Code Status is obtained and documented.
  • Participate in annual and complaint survey processes and assist with plans of correction.
