Life Care of Puyallup is a 4 and 5-Star quality-rated facility offering inpatient and outpatient rehab services, short-term and long-term care, and skilled nursing. The LPN Unit Care Coordinator is responsible for supervising, implementing, coordinating, and managing patient care through interpersonal contact with patients, families, nursing staff, and others on their respective unit, in accordance with all applicable laws, regulations, and Life Care standards. Life Care Centers of America facilities operate Skilled Nursing Facilities (SNFs) that provide long-term care for residents requiring ongoing skilled support and compassionate daily assistance, and short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury. This role involves collaborative care through an interdisciplinary team approach with nursing, therapy, and medical staff working together, serving a diverse patient population including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs. Patient-centered care is a top priority.
