Requirements

  • Licensed to practice medicine in accordance with Chapter 458, Florida Statutes plus four years of appropriate experience.
  • Completion of a residency program may substitute for the required experience.

Nice To Haves

  • Certification/Licensure in the state of Florida
  • Federal DEA Certification.
  • Medical Degree – M.D., D.O., or foreign equivalent.
  • Board certified in Family Medicine.
  • Completion of an accredited residency program.
