About The Position

We’re pleased to announce an opportunity for the position of TXIX Case Manager, within the Senior and Independent Living Services Department. This position provides federally regulated Medicaid case management services under Title XIX programs in compliance with federal Medicaid rules, Washington Administrative Code (WAC), and agency policy. The Case Manager is responsible for conducting comprehensive CARE assessments, authorizing long-term care services, issuing Planned Action Notices (PANs), and ensuring regulatory compliance. The role involves assessing client needs, developing and monitoring service plans, making appropriate referrals, and serving as a customer advocate with other service providers. Additionally, the position assists with department day-to-day operations as delegated, including reports, outreach, community events, support groups, and more. This role requires knowledge of related community resources and the laws and regulations specific to the area of service.

Requirements

Four years of paid, full-time, on-the-job social service experience in providing care management and long-term support and services to vulnerable adults, such as providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, and a high school diploma or its equivalent; or
Three years of paid, full-time, on-the-job social service experience, providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, a high school diploma or its equivalent, and are bilingual or bicultural with such experience; or
Two years of experience with other DSHS information systems used by Case Management staff, such as Comprehensive Assessment and Reporting Evaluation (CARE) tool, Barcode, ProviderOne, QA Monitor, ACES.Online, or ALTSA reporting, and college courses, an Associate’s degree, or its equivalent; or
Two years of paid, full-time, on-the-job social service experience and Bachelor’s degree; or
One year of paid, full-time, on-the-job social service experience and a Bachelor’s degree in social services, behavioral or health sciences; or
Social service experience and a Master’s degree.
One year full-time equivalent verifiable hours through an internship or volunteer work in social services may be substituted for one year of paid social service experience in providing care management and long-term support and services to vulnerable populations.
Part-time hours may be considered if the hours total one year full-time equivalent.
Individuals functioning as Case Managers prior to August 28, 1991, who have performed competently as determined by their performance evaluations, may be deemed Case Managers from a Washington State Area Agency on Aging when they meet the following educational requirements: High School diploma or its equivalent and four years of paid on-the-job social service experience prior to August 28, 1991; and Completion of state-sponsored CORE training.
Valid driver’s license in state of residence.
Auto insurance in the amount required by the State of Washington.
Successful completion of a criminal history background check.

Responsibilities

Provides federally regulated Medicaid case management services under Title XIX programs in compliance with federal Medicaid rules, Washington Administrative Code (WAC), and agency policy.
Conducting comprehensive CARE assessments, authorizing long-term care services, issuing Planned Action Notices (PANs), and ensuring regulatory compliance.
Assessment of client needs, development and monitoring of service plans, makes appropriate referrals and serves as a customer advocate with other service providers.
Assists with department day-to-day operations as delegated, including reports, outreach, community events, support groups, and more.
Complete the initial interviews with clients and their families to assess the customer's eligibility for services.
Gather relevant and useful data from the client, family, other agencies, to formulate a psychosocial assessment of the client and his or her family.
Assemble and guide group discussions and decision-making sessions among relevant professionals and program representatives, clients and their families, and significant others to formulate goals and design an integrated intervention plan.
Act as a liaison between the client and his or her family and all relevant professionals, programs, and informal resources involved in the overall intervention plan to help the client make his or her preferences known and secure the services needed.
Provide ongoing emotional support to the client and his or her family so they can cope better with problems and utilize professional and complex services.
