About The Position

Identifies, registers, and maintains a database of patients with malignant and non-malignant tumors, utilizing a tumor registry data system. Participates in the activities of data collection, editing, special studies, projects and follow-up and quality assurance. Assists in the analysis of data and disseminates information at a state and national level within guidelines, ensuring conformity of data.

Requirements

  • Must attain CTR certification within three years of employment as required by the American College of Surgeons Commission on Cancer.

  • One (1) year previous work experience as a CTR preferred or 1 year experience in a related health care field preferred.

Responsibilities

  • Maintains a reportable list including those diagnoses that are to be included in the registry and those that are excluded.
  • Maintains a computerized suspense file identifying cases to be entered into the registry.
  • Maintains a computerized accession register of all reportable cases accessioned in the registry.
  • Ensures accuracy of data collection on all registry cases for future patient care and research.
  • Reviews patient records, collecting 170 required fields of information as outlined in the current FORDS1, SEER Summary Stage 20182, AJCC3, Collaborative Staging, ICD-O4, STORE5, and New York State Department of Health Manuals.
  • Utilizes multiple sources when analyzing patient information to ensure accurate data abstracting.
  • Performs in-house reviews to assure quality of data and implement new data criteria.
  • Abstracts all core data elements according to the FORDS/STORE Manual.
  • Assigns proper topography and morphology codes according to ICD-0 coding classification system.
  • Assigns proper stage describing the extent of the reportable disease according to the AJCC Staging of Cancer, SEER and all current staging systems.
  • Enters all abstracted data information into the cancer registry software program.
  • Maintains a comprehensive method to identify and track patients due for follow-up regarding current health status.
  • Maintains a computerized system that identifies the annual follow-up date for each registry patient and determines those patients considered lost to follow-up.
  • Reviews patient re-admission records to collect current health status.
  • Reviews radiation reports to aid in determining a patient’s current health status.
  • Updates registry and follow-up files on all deceased patients.
  • Processes reports and updates all patient information on pre-existing registry patients.
  • Maintains a five-year follow-up rate of at least 90% and at least an 80% follow-up rate on patients in the registry since the reference date of 1980.
  • Utilizes hospital admissions computer system to track patients.
  • Sends patient and physician contact letters when current follow-up is not obtained.
  • Complies with departmental and Hospital-wide policies and procedures.
  • Complies with hospital confidentiality policies
  • Maintains expectations regarding an acceptable level of attendance.
  • Maintains a complete and accurate record of time worked.
  • Completes all hospital mandatory in-services on an annual basis.
  • Participates in group meetings regarding department subject matter.
  • Attends monthly department meetings or scheduled group meetings or reads and understands meeting minutes.
  • Also attends and reports at quarterly cancer committee meetings.
  • Attends department workshops/training sessions and educational seminars.
  • Certified Tumor Registrar attends educational conferences/seminars to maintain required certification and improve knowledge and professional expertise.
  • Responds to inquiries and requests of registry data in a timely manner.
  • Performs other related duties incidental to the work described herein.
  • Coordination of bi-weekly Tumor Conferences including agenda preparation, contacting physicians for case presentation, gathering clinical data, documentation and tracking of cases reviewed.
  • Completes COC recommended conference grid.
  • Prepares and submits conference evaluation forms and tracking sheets to the Rochester Academy of Medicine for CME credit for physicians.
