Requirements

  • Associate Degree in Nursing
  • Current, unrestricted Oregon RN license
  • Current, unrestricted Washington RN license (or ability to obtain within 6 months of hire)
  • Active BLS certification
  • National Provider Identifier (NPI) required, upon hire
  • 1-3 years related experience and/or training preferred in a Federally Qualified Health Center (FQHC) or primary care field is required.
  • Ability to work independently to the full ability of RN licensure.
  • Knowledge of the principles, practices and procedures of community health nursing, managed care, and consultation, including socioeconomic and cultural factors, methods and techniques of crisis intervention.
  • Ability to communicate professionally and effectively both orally and in writing with a 24/7 service-oriented approach, sometimes in stressful circumstances, with patients, coworkers, and the public.
  • Remains calm and effective in high pressure and emergency situations.
  • Must be able to display sensitivity to the population and patients served.
  • Ability to use good judgement in difficult situations.
  • Ability to comply with all HIPAA, OSHA, and Neighborhood Health Center policies and procedures.
  • Ability to learn and adapt to changing information and/or policies and procedures.
  • Ability to enter in data accurately with attention to detail.
  • Ability to work in a fast-paced environment.
  • Ability to answer and utilize multiple line telephone.
  • Must be able to demonstrate a record of reliable attendance and punctuality.

Nice To Haves

  • Bachelor of Science in Nursing degree
  • A minimum of 2 years of nursing experience is preferred.
  • Previous EPIC or EHR experience is preferred.
  • Ability to fluently speak, read, and write in Spanish is highly preferred.
  • Proficient use of the EPIC Electronic Health Record.
  • Working knowledge and proficiency of MS Office Suite (Outlook, Word, Excel) and desktop publishing software.
  • Knowledge of customer service principles and practices.

Responsibilities

  • Performs comprehensive health assessment of patients by completing the nursing process.
  • Identifies actual and potential health needs based on medical, mental/emotional, financial, and social factors.
  • Triages minor to emergent situations in person or telephonically to identify the nature and urgency of the patient's health care needs and then determines appropriate disposition, including scheduling of appointments in electronic medical record.
  • Responds to medical emergencies within the clinic setting.
  • Completes all in person and telephonic interactions in a timely, professional manner with courtesy and respect.
  • Manages an Epic in-basket of patient calls, MyChart messages, test result notifications, and other messages that directly impact patient care plans.
  • Provides care for patients using NHC approved clinical protocols.
  • Assist patients to maintain maximum quality of life by coordinating services of community agencies and encouraging preventive practices.
  • Assist patients to establish short and long-term goals to obtain optimum level of health through advocacy, counseling, teaching, problem solving, and utilization of resources.
  • Consults and coordinates with health center team members to develop and implement comprehensive patient care plan based on assessment.
  • Performs RN led patient visits for health maintenance, patient education, and treatment of acute issues based off protocols and provider orders.
  • Administers medications and immunizations as ordered by an authorizing NHC provider or via NHC approved clinical protocols.
  • Instructs individuals, families, and other groups on topics such as health education, disease prevention, and develops health improvement programs.
  • Monitors, plans, implements, and evaluates coordination and management for patients with chronic illnesses.
  • Accurately, concisely, and succinctly document all patient contacts in the electronic medical record.
  • May assist with phlebotomy, CLIA waived point of care testing, in clinic EKGs, and other primary care procedures as necessary in times of staffing constraints.
  • Actively participates in the management of the clinic vaccine program.
  • Implements required nursing procedures consistent with NHC procedures/protocol.
  • Participates in process and quality improvement workflows.
  • Assists in new employee orientation, onboarding, and training.
  • Participates in nursing team and clinic daily huddles at the start of the workday.
  • Performs other duties as assigned.
