About The Position

VNA Hospice NWI is hiring a full-time Triage RN for Home Hospice. This role involves providing professional nursing support via phone to patients, families, and callers, dispatching on-call staff and the hospice care team for in-person support, and coordinating after-hours service delivery. Responsibilities include patient and family education, dispatching interdisciplinary visits, coordinating with external vendors, and providing initial bereavement support. The triage nurse documents all interactions in the patient record, promotes a positive customer service approach, and acts as the first line of support outside regular business hours, initiating the intake process as needed. The role also involves supporting staff and working in the field when necessary, initiating the complaint process, and reporting to the supervisor on call.

Requirements

  • Must have and maintain license as a registered nurse by the state of Indiana.
  • Minimum of associate degree in nursing, graduate of a school of professional nursing.
  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care.
  • Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid Indiana driver’s license and auto insurance.

Nice To Haves

  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.

Responsibilities

  • Supporting patients, families, and all other callers by providing professional nursing support via the phone.
  • Dispatching on-call staff and other members of the hospice care team to provide in person support when needed.
  • Coordinating and prioritizing all service delivery after hours including patient and family education, dispatching of interdisciplinary visits, and coordination of services with external vendors and resources as needed.
  • Providing initial bereavement support to families as needed.
  • Documenting interactions with patients, families, contacts, and vendors in the patient record to include the assessment, caller agreement to the plan, any actions and interventions and the resolution of each patient/family interaction.
  • Promoting a positive customer service-oriented approach by being a calming presence and supporting voice for patients and family members in their time of need.
  • Serving as the first line of support for those needing assistance outside of regular business hours and initiating intake process as needed to ensure streamlined access.
  • Supporting staff members and working in the field as needed.
  • Initiating the complaint process as needed per organization’s policy.
